Plan Options

Care Endocrinology offers flexible plan options designed to accommodate various patient needs, emphasizing personalized care and convenience. Below is a detailed overview of each plan, including key features and pricing. For comparison, refer to the table summarizing costs and benefits.

• Direct Pay - Standard Patients pay the provider directly in full for services, with insurance claims handled independently by the patient. We provide a superbill for submission to your insurer. Benefits include minimal or no office waiting time, prompt appointment starts, advanced telemedicine connections (via secure video platforms with integrated health records), direct provider access without referrals, and guaranteed responses to non-urgent queries within one to two business days.

  • New Patient Visit: $295 $225 Limited Availability

  • Follow-up Visit: $185

• Direct Pay - Membership This membership plan includes all features of the Standard plan, plus longer, unrushed appointments to thoroughly address endocrine concerns, direct telephone and text access to the physician between visits, and priority appointment scheduling. Patients may call to request an earlier appointment if no suitable slots are available on the schedule.

  • New Patient Visit: $440

  • Follow-up Visit: $275

  • Membership: $50/month, billed semiannually (every 6 months)

Additional Policies

• Telehealth Follow-up appointments may be scheduled via telehealth, provided patients are physically located in the state of Illinois at the time of the appointment. Initial consultations are required to be in-person. Technical requirements include a stable internet connection and a device with camera and microphone capabilities; our office can provide guidance on setup if needed. Accommodations for patients with disabilities are available upon request.

• Medicare Medicare beneficiaries may be seen under a private contract and cannot submit claims to Medicare for reimbursement. Dr. Trendafilova has “opted out” of Medicare for her services.  Some supplemental insurance carriers may provide some reimbursement for patients seen by a non-Medicare physician.  You can still use your insurance as you normally would, to pay for labs, medications, imaging and referrals that she orders.

• Commercial Insurance Plans Dr. Trendafilova is considered an out-of-network provider with commercial insurance carriers. Following your visit, we will provide a superbill for you to submit to your insurer for potential reimbursement. Please consult your insurance carrier regarding out-of-network benefits. You may continue using your insurance as usual for laboratories, medications, imaging, and referrals ordered by the provider.
